Actually, in the Middle East and Asia, toilets are customarily flush with the floor, so people are accustomed to their rear ends hovering above the toilet hole. Therefore, signs like this are necessary to inform the "locals" how to use Western-style toilets.

Medically speaking though, the first picture is a healthier way to go.Bending you body like that straightens out the colon and you release more stool.The other way that we all use puts a kink in the colon, where cancer is most lkely to start. When you go sit with your shoulders lowered to almost your knees.Just don't shit on the wall behind you. :D
